Stryker Warren

STRYKER WARREN, JR. serves as President, Chief Executive Officer and a
Director of Urology Healthcare Group, Inc. Mr. Warren’s healthcare
tenure of twenty-six years includes senior management responsibilities
in the prepaid healthcare industry with INA Healthplans (now CIGNA
Healthplans) and HealthAmerica Corporation. Mr. Warren participated in
the acquisition of Healthplans as well all aspects of de novo
development. He has served as CEO of several HMOs and as the President
of HealthAmerica’s subsidiary development company. Subsequent to his HMO
involvement, Mr. Warren founded two venture capital-financed specialty
companies: FOCUS Healthcare Management, a specialty workers’
compensation managed care entity which was sold to United Healthcare
Corporation and Meridian Occupational Healthcare Associates, a
consolidator of occupational medicine practices. He served as founder,
Chairman and CEO of both FOCUS and Meridian; he is currently Chairman of
Meridian. Mr. Warren graduated from Oregon State University in 1972 with
B.S. in Pharmacy and was born in 1949.
