G. Richard Smith – Coronado Industries Inc (cdik)

G. RICHARD SMITH has been a Director of the Registrant since November 5, 1996 and Secretary of the Registrant since November 5, 1996. He became Chairman in March, 1998. From July, 1995 to November 5, 1996 Mr. Smith was a joint venture partner in American Glaucoma, the company that developed the concept of glaucoma treatment centers throughout the US which was sold to the Registrant. From July, 1995 to November 5, 1996 Mr. Smith was a member and President of Ophthalmic International, L.L.C., the company that developed and patented the glaucoma treatment which was sold to the Registrant. From 1987 to June, 1995 he was co- owner and President of Southern California Medical Distributors, Ltd. ('SCMD') which developed a turbine powered keratome for eye surgery.
